- Browse and select the file you want to upload
- In the description, please include:
- your Full Name,
- Date of Birth
- University ID if available/applicable,
- A brief description of the file being uploaded
- who requested you to upload the file if applicable.
- Please enter an email address that we could use to reach you with questions about your document if necessary
